Complete the DS-160 online and pay the visa application fee. The Mission page states USD 190, non-refundable, but that page is dated March 2021, so check the current fee before you pay. Interviews are held at the US Consulate General in Johannesburg, Cape Town or Durban.
Be completely honest about any previous arrests or earlier time in the United States. Concealing them will result in denial of the visa and may lead to permanent ineligibility.
